After providing their zip code, applicants are shown the plans that are available for them in their area.  To purchase, they need only enter who needs to be covered, home address, phone number, gender, birthdate and email address along with a valid form of payment.

Plans

Through Guardian Direct, consumers have access to three tiers of PPO dental insurance plans. The first, Bronze, is for people who are only expecting to get cleanings and the occasional filling. The PPO plan tiers' offering is as follows:

I did not have a good experience with this insurance. Purchased it through the government website for insurance. Apparently I failed to select a "primary care" provider when I signed up. I was not notified, instead a provider was selected for me, again I was not notified. So, I went to a different provider, one in-network and closer to my home. I went twice there twice this year, the last time was in October. Soon I was notified that since I went to a different in-network provider than the one that was assigned to me, they will not cover the costs of those visits. They were very happy to accept my premiums though, and told me I would be covered going forward.

I have no idea how this company is rated 4.8 stars. My employer switched to Guardian this year and they are a nightmare to deal with, everything is denied even when you have the "best" plan. My dentist decided to come up with his own insurance plan to try and get away from dealing with Guardian. It's too bad we can't re-elect benefits until July 1st because I'd rather have no coverage than have Guardian at least then I'd stop getting denial letters and just know I had to pay everything out of pocket. How frustrating, buyer beware.

Their customer service is terrible. They canceled my dental insurance without notifying me and I have been making payments to them. I got through ACA and they have told me to call the exchange but my payments have been going to Guardian. I call the "Exchange" aka ACA. They told me it's Guardian that not process my request to reinstate and they handling the billing. A few months ago they said they would take care of it......but they didn't. According to them I'm still not reinstated because of one late payment. These past few months I have been making payments to them for the dental insurance. So now that I need to go to the dentist I don't have insurance but I've been paying them. What... Read More

DO NOT BOTHER PAYING FOR THIS WORTHLESS INSURANCE!!!! THEY WILL TAKE YOUR MONEY AND PROVIDE YOU WITH NO BENEFITS. I AM THREE WEEKS INTO TRYING TO SIMPLY GET A SECOND OPINION BEFORE I PROCEED WITH SIGNIFICANT PROCEDURES RECOMMENDED. I HAVE TO GET APPROVAL BEFORE I CAN EVEN GET A SECOND OPINION, WHICH ONE WOULD THINK THEY'D APPRECIATE BEFORE SPENDING THEIR MONEY, BUT I'M GUESSING THEY DON'T ACTUALLY PAY THE DENTISTS SHIT FOR THEIR WORK, THE DENTIST ONLY GETS WHAT WE PAY, NOTHING MORE. THEY CLAIM THAT I DIDN'T PROVIDE THEM WITH THE NECESSARY CODES WHEN I CALLED TEN DAYS AGO, WHICH IS BS, SO NOW I HAVE TO WAIT ANOTHER 3-5 DAYS FOR THE PROMISED APPROVAL, AND I'M SURE THEY'LL COME UP WITH YET... These people are frauds. I bought a policy that claim 100% coverage for preventative services and it was a lie! When my doctor called, they said the services for teeth cleaning and minor x-rays were covered. Then I got the EOB, and now I owe a $60 deductible! That is after paying $14/mo. So I still pay $228 for the services + deductible which would have been about the same amount I would have paid without insurance. Horrible!

HORRIBLE! HORRIBLE! DO NOT buy insurance with this company. They are liars and are unethical. I had a routine exam and a filling on May 15, 2019 and the dentist filed the claim. Nothing. I called in June two times and spoke with "David" and then "Kylie" - these are all people in India or somewhere not in the US. I was told each time that they never received the claim. My dentist resent the claim. I waited. Nothing. By now I knew that I had better document. I called for the THIRD time on 7/22/19 and spoke with "Von" who called the dentist's office. I was told that the claim was sent directly to "Von" and I needed to wait 15 business days. I called for the FOURTH time on... Read More

If you would like a "dentist" for Guardian to review your claim and then deny it, despite all the evidence to the contrary, then please use Guardian. What a joke of an insurance company. Will be switching promptly.

If you would like to pay a company to reject necessary tests and services, choose Guardian. If you would like to be ignored when waiting on the phone forever, choose Guardian. If you want your health to decline whilst being defrauded by a group of New York con men, choose Guardian. I will be getting new insurance tomorrow. I shall continue to inform people about this pathetic and amoral excuse for a dental insurance provider for the next decade. Be watching you "Guardian".

I've been trying to get my problems with this insurance company. They ave no problem taking my money for their insurance company. When it comes to verifying my coverage they are clueless. I can imagine what it would be if my dentist was being difficult. I believe it is time for "social media" intervention should be as to their online reviews

Very awful dental insurance. Did not cover even 10 percent of my dental cost. Total waste.
