Consumers can locate a vision insurance provider by searching through EyeMed Vision Care’s website. To enjoy these benefits, consumers must first register and enroll to secure a membership. The member then presents their plan ID and Member ID number to their chosen provider. No claim or authorization forms is required for in-network benefits.

In-Network Inclusions:

• Contact lens wearers will pay up to $55 for standard contact lens exam which includes fit and follow-up. Receive 10% off retail price for premium contact lens exam, fit and follow-up.
• $20 co-pa on eyeglass lenses once every 24 months. Plans cover standard plastic single vision, bifocal or trifocal lenses of any size and power.
• $20 co-pay on contact lenses once every 24 months. Plans include a $130 retail allowance which can be applied toward the purchase of conventional or disposable contact lenses. In case a member selects conventional contact lenses with a retail price of over $130, they will receive 15% off the balance.
• $0 co-pay on frames once every 24 months. Plans include $130 retail allowance which can be applied toward the purchase of any frame available at the provider location. The client will receive 20% discount off the balance if they choose a frame over $130.

• 15% off additional purchases of conventional contact lenses
• 20% off non-covered items like cleaning cloths or non-prescription sunglasses
• 40% off additional complete pairs of eyeglasses
